The following leads you through the step-by-step forms that will need to be signed or filed with the appropriate authorities.

  1. GENERAL PRICE LIST

    If you decide that you want us to be your funeral director, we are required by law to give you a copy of our General Price List before discussing cremation arrangements. You may download and print a copy of our General Price List.

  2. CREMATION AUTHORIZATION

    If you have decided on cremation either for yourself in the future or for a family member who has just died, we are required to have a cremation authorization. A living person may sign his/her own cremation authorization. The next-of-kin sign the cremation authorization for a deceased person. Both require the signature of a witness. Fill out the Cremation Authorization Form.

  3. DEATH CERTIFICATE WORKSHEET

    After a death occurs, and before a cremation can be performed, a death certificate is prepared by the funeral director. Signed by the attending physician or other authority, and filed at the Department of Health. There is a lot of information required on the death certificate.   4. CONTRACT OF GOODS AND SERVICES

    Before we can perform a cremation, we are required to furnish you a signed contract clearly stating the cost of services to be performed. The sample contract is not valid until physically signed by all parties. Begin filling out the sample contract.
